Adult Day Care Social Worker
Oneida Adult Day Care is actively seeking an experienced Social Worker for our Skilled Nursing Facility in Utica New York.
Must have social work nursing home experience
DUTIES:
Hours: 8am 1:30pm Monday through Friday
Oneida Adult Day Care is actively seeking an experienced Social Worker for our Skilled Nursing Facility in Utica New York.
Must have social work nursing home experience
DUTIES:
- Maintaining and/or improving residents ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the residents admission community reentry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Bachelors Degree in Human Services; Psychology or Social Work
Hours: 8am 1:30pm Monday through Friday
