About the role:
The LVN SNF Liaison plays a crucial role in delivering quality mental health services within SNFs. They build and maintain strong relationships between EMH s clinicians and SNF management teams ensuring a continuum of care for residents. Liaisons excel in customer service and problemsolving ensuring that interactions are handled with professionalism and empathy.
Key Responsibilities:
- Support SNF Management and Clinical Team:
- Build and maintain relationships with SNF management and staff to ensure the highest quality of mental health care.
- Coordinate and participate in meetings audits and surveys to ensure compliance with regulatory standards and Physician Orders.
- Facilitate inservices trainings and psychiatric medication reviews to support both SNF staff and EMH clinicians.
- Communicate effectively with clinicians and SNF staff managing patient referrals and eligibility and ensuring that patients are appropriately added to caseloads.
- Provide onsite support to clinicians addressing issues increasing referrals and ensuring smooth operations.
- Act as a liaison between EMH and SNFs providing daily updates to management and ensuring that all relevant information is communicated to the appropriate parties.
Requirements
- Valid Licensed Vocational Nurse (LVN) credentials.
- 23 years of experience in skilled nursing or hospital settings with a strong understanding of CMS survey preparation.
- Strong interpersonal communication and organizational skills with a focus on customer service and time management.
- Willingness to travel within Los Angeles County and the San Fernando Valley with reliable transportation.
- Familiarity with electronic health records medical terminology and HIPAA regulations.
