Home Health - RN

We are seeking a compassionate and experienced RN to join our extraordinary care team. This fulltime position involves providing management and delivery of patient care plans in Carnegie/Crafton PA and surrounding areas. The role requires oncall availability on weekends and local travel to patient locations.

Schedule and Shift Details
Oncall Friday Saturday and Sunday from 5:00pm to 10:00pm 8 units on Saturday 8 units on Sunday 40hrs/week

Key Responsibilities
Lead and collaborate to provide management and delivery of patient care plans
Support patients in Carnegie/Crafton PA and surrounding areas
Spend 20% of the day driving to/from patient locations
Consistently meet critical deadlines for charting
Selfmanage time and schedule

Required:

Valid Registered Nurse(RN) in the state of Pennsylvania(PA)

Associate Degree or higher with 1years of experience in ANY of the following:

1. Clinical Documentation
2. Clinical Informatics
3. Clinical Quality

• 1years of experience in ANYof the following:

1. Electronic Media Record(EMR)
2. Other EMR
3. Practice Partner EMR

ALL of the following valid licenses/certifications:

1. American Red Cross Advanced Life Support(ARC ALS)
2. American Heart Association Advanced Cardiac Life Support(AHA ACLS)
