Job Description

Position: Case Manager
Type of Position: Temporary (Due to end September/October)
Scheduled: Monday Friday 8:00 am 5:00 pm (Weekends if necessary)
Pay:$20.00 $23.00/hour
Location: Houston TX 77054

Smarter HR Solutions is looking for individuals who exhibit great critical thinking skills and are knowledgeable in HUD grants to fulfill our contract Case Manager role. This role is focused on addressing the housing infrastructure and public service needs of the low to moderateincome community in Harris County. This is accomplished through a process of securing and managing grants from the U.S. Office of Housing and Urban Development (HUD) and partnerships with other government and nonprofit agencies resulting in a variety of programs projects and services. The Case Manager will assist clients with a range of concerns including but not limited to housing stability needs financial assistance location of shelter or housing job preparedness etc.

Job Responsibilities
  • Completes inperson interviews to address a degree of problems through the completion of comprehensive assessments.
  • Secures information such as medical and social factors that contribute to the client s situation.
  • Often assist the clients who have issues including but not limited to neglect abuse domestic violence mental health hopelessness chemical addiction HIV trauma child protective services and parental substance abuse.
  • Extensive documentation and data entry is required.
  • Entering information in the database including case notes assessments email reports etc.
  • Communicating with agencies including the Housing Authority and Houston Coalition for the Homeless.
  • Completing required paperwork per Harris County procedures and HUD regulations.
  • Infield workincluding but not limited to phone and/or virtual meetingsmonthly home visits phone and/or virtual visits with clients landlord contact contact with Housing Authority social service agencies check delivery housing search meet with clients at encampments meetings trainings attend court shelter visits etc.
  • Attend meetings trainings communication with direct reports reading email reading policy and procedures completing reporting and preparing files for audits.
  • Locating resources in the community referring clients and linking clients to services.
  • Disaster related assignments as needed.


Requirements

Knowledge Skills and Abilities
  • Knowledge of the NASW Code of Ethics as a guide to decisionmaking and promote ethics and values to the social work profession.
  • Knowledge of the Fair Housing Act.
  • Ability to engage clients and exhibit compassion.
  • Ability to use critical thinking skills to assess and assist clients.
  • Ability to work with the homeless.
  • Valid Drivers License (Texas upon hire)
Physical Demands
  • Some sedentary work driving to complete field work including driving a Company car and/or personal vehicle.
  • In field work can include but not limited to home visits landlord visits agency visits verify homeless status in outside elements up and down stairs at apartment complexes.
Required Education/Experience
  • Bachelor s degree in Social Work or related field.
  • Two (2) years of experience working with vulnerable populations or the homeless.
  • Extensive documentation and data entry is required. Preferred
  • One (1) year of experience working in the Child Protective Services sector.
  • One (1) year of experience working with foster children and/or families involved in the CPS system.
  • One (1) year of experience working with any HUD grant.
  • Trained in TraumaInformed Care.
  • Trained in Motivational Interviewing.

Requirements: Proficiency in the use of Microsoft Office Windows, Excel, Access, PowerPoint and Word software. Excellent verbal, interpersonal and written communication skills. Strong planning and organizational skills. Self-starter and ability to work independently. Demonstrate an outstanding level of professionalism in providing administrative support. Ability to exercise good judgment, discretion, tact, and diplomacy. Ability to identify existing or potential problems and recommend solutions. Excellent problem-solving skills with ability to analyze situations. Possess excellent attention to detail to maintain quality control over assignments and produce error-free work. Ability to learn applicable software programs. Ability to read and interpret regulations pertaining to the administered programs. Valid driver's license, reliable transportation, and liability insurance. Preferences: Associate's Degree One (1) year experience working in an office setting. Experience working with programs funded with HUD's Community Development Block Grant program and/or FEMA disaster recovery funds. Environmental & Physical Requirements: The work is primarily sedentary with occasional required travel to training events or meetings. The work is performed in a professional office setting. No special safety precautions are required. Ability to lift up to 10 pounds. Ability to bend and stand as necessary.
