Job Description
Adelphi is seeking a Licensed Bachelor Social Worker (LBSW) to provide services at a facility in Osawatomie Kansas. The ideal candidate will deliver direct patient care services for adults receiving inpatient psychiatric treatment.

Requirements:
Bachelors Degree in Social Work
Active KS license
Valid Drivers License
One year experience with psychiatric patients preferred
Responsibilities:
Meet with patients and the interdisciplinary team to provide updates to family and community partners.
Gather and document information in the social service assessment and discharge plan.
Create safe and healthy discharge plans.
Coordinate specialized aftercare.
Serve as point of contact for patients wellbeing with members not on the care team.
Act as the key informational flow regulator regarding a patients progress in treatment.
Perform care consistent with the scope of licensure and in accordance with the facilitys policies and procedures.
Work effectively with fellow medical staff and treatment team members.
Any other duties as assigned.
