Job Description
Responsibilities:
- Build and maintain relationships with external facilities.
- Conduct outreach to secure patient referrals.
- Collaborate with local sources to expand our network.
- Enhance community presence through strategic partnerships.
Qualifications:
- Strong communication and interpersonal skills.
- Experience in outreach and network building.
- Ability to work independently and as part of a team.

Hybrid work model: In person and Remote
Licensed in New York State (bonus if licensed in multiple states) with a Psychiatric Mental Health Nurse Practitioner Certification. (BC-PMHNP) Experience in treating a wide array of psychiatric conditions. Experience with adult, adolescent, geriatric, or child populations. Preferable to have 10+ years experience Comfortable working in a hybrid environment (mix of in-person and telehealth)
