Responsibilities:

• Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans tailored to individual patient needs.
• Conduct thorough physical examinations and document patient medical histories.
• Diagnose and treat acute and chronic illnesses prescribe medications and manage overall patient health.
• Order and interpret diagnostic tests including lab work and imaging studies.
• Provide comprehensive patient education on disease management preventive care medication adherence and healthy lifestyle choices.
• Develop and oversee patient discharge planning to ensure continuity of care.
• Serve as a patient advocate coordinating care and resources to ensure optimal health outcomes.
• Maintain accurate and timely electronic medical records in compliance with healthcare regulations and standards.
• Participate in quality improvement initiatives and contribute to the development of clinical protocols and best practices.
• Mentor and supervise nursing staff providing guidance and support as needed.

Requirements:

• Valid Nurse Practitioner license.
• Masters degree in Nursing with specialization as a Nurse Practitioner.
• Certification as a Nurse Practitioner.
• Excellent communication skills to effectively interact with patients families and healthcare team members.
• Ability to lift and move patients as needed.
