Position:
Primary Care Physician

Responsibilities:

• Perform comprehensive inperson assessments and provide primary care to participants.
• Collaborate with communitybased physicians and IDT to develop and implement care plans.
• Oversee care in nursing homes and hospitals including managing admits and discharges for acute shortterm and longterm placements and coordinating the use of medical specialists.
• Participate actively in IDT meetings communicating changes in participant status collaborating on care planning decisions and ensuring effective 24hour care delivery.
• Make regular visits to maintain participant care in nursing homes as needed.
• Participate in rotating night and weekend call.

Requirements:

Required Qualifications:

• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education (ACGME).
• Doctorate Degree (MD or DO).
• Board Certification in Internal Medicine or Family Practice.
• Active California Physician License without Restrictions.
• State CDS Certification and DEA Registration.

Preferred Qualifications:

• Board Certification in Geriatrics.
