Position:
Nurse Practitioner Primary Care Provider

Responsibilities:

• Conduct inperson comprehensive assessments including initial semiannual and unscheduled assessments and provide continuous medical coverage to participants.
• Collaborate with physicians and IDT to develop and implement care plans for each participant.
• Participate in discharge planning from acute or longterm settings in collaboration with physicians and IDT.
• Maintain participant care in nursing homes with regular visits as needed.
• Work with WelbeHealth physicians communitybased PACE physicians and IDT to ensure effective care coordination.
• Review outcomes and utilization data including hospitalizations and specialist use to establish best practices and policy improvements under the Medical Directors guidance.
• Participate in rotating night and weekend call.

Requirements:

Required Qualifications:

• Graduation from an Accredited School of Nursing.
• Certificate as a Nurse Practitioner with Prescriptive Authority.
• RN State License.
• Minimum of two years of experience working with the senior population.
• Current knowledge of chronic care/geriatric issues and best practices.
