Care Manager II RN

Job Title: Care Manager II (RN)

Duration: 6 months (Potential to Extend)

Location: San Antonio TX

Shift: Day 5x8Hour (08:00 05:00)

Notes:

• Remote Preferably residing in the San Antonio/Bexar area with availability to travel to surrounding areas (within 2 hours). Most memberfacing meetings will be at the homes of members within the San Antonio area.

Primary Responsibilities:

• Perform administrative and clerical duties related to supporting health plan operations.
• Provide care management services to members ensuring they receive appropriate care and services.
• Conduct member assessments and develop care plans tailored to individual needs.
• Coordinate with healthcare providers community resources and support services.
• Travel to member homes for inperson visits and assessments.
• Performance Expectations/ Metrics:
• Maintain high productivity in task completion and assigned duties.
• Ensure timely and accurate documentation and reporting.

Day One Overview:

• Integration into ongoing team activities and responsibilities.
• Begin member interactions and administrative tasks.
• Background and Experience:
• Experience in managed care settings.
• Bilingual abilities are a plus.
