Job Description
Job Title: Registered Nurse Case Manager
Location: Apple Valley CA 92307
Shift: Day 5x8Hour (08:00 16:30)
Block Schedules: No
Start Date: 07/01/2024
Job Duration: 13
Duties:
- Review per customized Clinical Track listed below:
- Admission Criteria (InterQual)Competent InterQual guidelines.
- Experience with Assessing patients Care Coordination Discharge Planning and Utilization Review Ability to organize and participate in family meetings.
- Participate in the development of the treatment plan.
- Experience with HMOs IPAs etc.
- Knowledge of adoption/foster care drug/alcohol abuse treatment options patients experiencing homelessness patients with mental health history or in crisis advance directive POLST protective services process etc.
- Competent with arranging DME SNF and ambulance transport. Must be able to perform this task independently.
- Required:
- 2 years of Acute Care experience (can consider 1.5 years)
- Experience with Utilization Review and Discharge Planning
- Experience with InterQual
- RN License from CA state.
- BLS
- COVID Card
