Job Description

vTech Solution Inc is hiring an Enrollment Case Manager in Allegheny and Berks County of PA

About us:
Duration: 6 months (High possibility of extension)

Pay rate: $20.5/hr.

Notes:

  • There may be instances where ECMs are expected to and scheduled to travel outside of this radius as dictated by business needs.
  • Travel expenses will be reimbursed.
  • Must have own vehicle and valid DL.
  • The Enrollment Case Manager will be scheduled (in most instances) throughout the day with appointments that have a travel time of 45 minutes or less from their prior location usually within their assigned county.
  • There will be 2 weeks training in Harrisburg PA 17106
  • ECMs work primarily in applicants places of residence.
  • They will conduct intake visits for waiver applicants and act as a facilitator for the eligibility determination.

Job Responsibilities:

  • Travels throughout assigned county and neighboring counties as dictated by business needs working primarily in applicants places of residence.
  • Conducts intake visits for HCBS applicants and acts as a facilitator for the eligibility determination process providing assistance from the time individuals are identified as potential participants in an HCBS program to the time they are enrolled as participants or are terminated from the intake process.
  • Presents and discusses the concept of HCBS programs and advises applicants of their rights and responsibilities.
  • Assists with application submission including data entry of the Compass Application
  • Assists applicants with selecting an MCO Life Provider or Supports Coordinators that best meet their needs ensuring consumer control is maintained throughout the application process.
  • Assists with developing relationships with communitybased organizations advocates and stakeholders to gather feedback on improving the application process and removing barriers to enrolling in HCBS waiver programs.
  • Fosters and maintains relationships with parties serving our mutual consumers including physicians County Assistance Offices providers nursing homes and rehabilitation facilities.
  • Facilitates the efficiency of the application process ensuring any stalled cases receive extra attention and support.
  • Attending regularly scheduled meetings.
  • Reports on schedule availability at regular intervals
  • Occasionally lifts carries or otherwise moves items weighing up to 25 pounds.
  • Performs other duties as assigned by management.

Qualification Preferred:

  • Experience in public or private social work.
  • Case work experience or experience performing assessments of individuals functional ability to determine the need for institutional or communitybased services.
  • Bachelors degree with a social welfare major; Or any equivalent combination of experience or training with successful completion of a minimum of 12 semester hours credit in sociology social welfare psychology gerontology or other related social science.
