Job Description
Registered Nurse Case Manager
Location: Burbank CA
Duration: 13 weeks
Shift: Night Day 5x8Hour (08:30 16:30)
Responsibilities of Registered Nurse Case Manager
- Conduct diagnostic evaluations and assessments to determine patient needs and develop individualized care plans.
- Provide direct patient care including administering medications monitoring vital signs and assisting with procedures.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and ensure continuity of services.
- Educate patients and their families on disease management treatment options and selfcare techniques.
- Monitor patient progress and make adjustments to care plans as necessary.
Requirements for Registered Nurse Case Manager
- RN License
- 1 year experience in Case Management
