Shift Notes: 8hr days (7a3.30p or 8a4.30p) 40hr guarantee

Weekend Requirements: Every fourth weekend/Holiday coverage is required if scheduled

OnCall Requirements: None

COVID Vaccination: COVID VACCINATION (dose date on or after 10/3/23) NOW REQUIRED FOR THIS HEALTH SYSTEM. Exemption forms are available in Downloadable Forms (Both forms REQUIRE Provider/Religious Leader Signatures) or please ask your delivery team for documents. IF APPROVED BY THE HEALTH SYSTEM TRAVELER WILL WEAR A MASK AT ALL TIMES WHILE ON SITE. To avoid delays in processing please include the travelers COVID vaccination card/or intent to fill out the exemption form with profile submission.

Traveler Policy: Local Travelers who reside within 50 miles of the facility will not be considered. PERM ADDRESS MUST BE UPLOADED WITH PROFILE.

Dress Code: Personal scrubs of any color

**AT LEAST TWO SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW MUST BE ADDED TO THE PROFILE PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION. ANYTIME WILL NOT BE ACCEPTED.

**Requested time off during this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!! NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better).

**Certified Wound/Ostomy/Continence Nurse (WOCN) required

• 2 years of current wound care experience
• Certified Wound/Ostomy/Continence Nurse (WOCN) required NON NEGOTIABLE! MUST BE IN HAND AT PROFILE UPLOAD!
• Meditech Expanse experience is strongly preferred

• 1 Supervisory Signed Reference (from within the past two years of employment)
• License/Certifications Must be uploaded with profile

• ACLS (AHA)
• BLS (AHA)
• Wound Care Certification
