Job Description

Job Title: HCS Case Manager


Location: Victoria TX

Job Type: FullTime

Position Overview:


We are seeking a dedicated and compassionate HCS Case Manager to join our team in Victoria Texas. The ideal candidate will oversee the coordination and administration of all aspects of an ongoing program to serve the needs of individuals with disabilities. This role involves ensuring compliance with the Texas HCS program regulations developing individual program plans and working closely with families and healthcare professionals to provide exceptional care and support.


Key Responsibilities:


  • Conduct assessments to determine client needs and develop individualized service plans.
  • Monitor and evaluate the effectiveness of service plans and make adjustments as needed.
  • Advocate for clients needs and rights within the community and various healthcare settings.
  • Coordinate with healthcare providers family members and other stakeholders to ensure comprehensive care.
  • Maintain accurate records and document client progress.
  • Ensure compliance with all federal state and local regulations concerning HCS services in Texas.


Qualifications:


  • Bachelors degree in Social Work Psychology or related field preferred.
  • Minimum of two years experience in case management preferably within an HCS environment.
  • Strong understanding of Texas HCS program regulations and guidelines.
  • Excellent communication and interpersonal skills.
  • Proven ability to manage multiple priorities in a highpaced environment.
  • Bilingual (Spanish/English) skills are a plus.
