We are currently hiring a contract School Social Worker

K12^th Grade Caseload

Daily Expectations: Provide social work support.

Full Time Hour 37.5 Hours

Essential Functions:

Serve as a team member for programs as assigned.

Participate in the development of curriculum with particular emphasis on socialemotional behaviors.

Provide referral and prereferral consultation.

Conduct an evaluation to discover socialemotional needs and use the findings in the development of an FBA or functional behavior assessments or Behavioral Intervention Plan.

Ensure that an effective treatment plan has been developed for each student served.

Provide regular consultation to program staff and parents in matters pertaining to the socialemotional needs of students.

Participate as a member IEP Team (Individualized Educational Planning Team)

Maintain confidentiality

Write appropriate present level of performance statements with current data.

Provide appropriate documentation of programs of goals and objectives.

Provide direct services to students and/or parents as described in the IEPT meeting report.

Establish time in daily/weekly/monthly schedule for the purpose of: a. Conducting student evaluations b. Providing direct services to children/youth c. Providing direct services to families d. Attending IEPT meetings and e. Participating in program development and inservice training activities.

Maintain prepare and process the district s required documentation which includes but not limited to the following: Program data Medicaid student count etc.

. Maintain student records student objectives and security/inventory of equipment and materials according to identified system.

Requirements

Minimum 1 years social worker experience required.

Master s degree in social work.

Illinois State License in LSW or LCSW

Illinois Professional Education License (PEL) endorsed in Social Work

Ability to work collaboratively in a multidisciplinary team.
