Join our clients missiondriven team as a Complex Care RN for the PACE Program in downtown Los Angeles. The ideal candidate will be passionate about delivering highquality integrated care to frail older adults and will play a critical role in the management and delivery of direct nursing care to PACE participants. Responsibilities
- Provide direct medical care to PACE participants
- Complete sections of assessments and care planning
- Manage a panel of participants
- Initiate evaluate review and change orders for medications and diagnostic studies or treatments with approval
- Assess initiate and coordinate care on a daily basis under supervision
- Document all participant interactions assessments next steps in the EMR within the specified timeframe set by the center
- Make referrals to specialists with approval and request consults as needed
- Perform history and physicals and participant teaching
- Order interpret and follow up on lab tests and results
- Perform selected procedures
- Recognize and escalate abnormalities as needed
- Liaise with other team members for participant s care including behavioral health and pharmacy
- Evaluate and treat participants where they are located whether that be in the center home virtually SNF etc
- Facilitate advanced planning discussions with participants
- Document chronic conditions via HCC capture
- Provide oncall coverage as needed
Requirements
- Unrestricted California Registered Nurse license
- Graduate of accredited RN program; BSN strongly preferred
- Fluency in Spanish
- CPR and First Aid certification (within 60 days of employment)
- Alzheimers certification preferred
- Trained in wound care IVs blood draws
- Passion and mission orientation for serving highrisk seniors and frail older adults
- Minimum 2 years of experience caring for adults chronically disabled or frail or elderly populations; Ambulatory care experience is helpful but not essential
- Ability to function independently in the daytoday management of the clinical needs of participant and the operations of the PACE Center Clinic utilizing the company s policies and procedures leadership guidance and program goals in the decisionmaking process
- Active listening and conflict management skills
- Ability to adapt quickly to multiple clinical settings
- Ability to lift and carry or otherwise move up to 50 pounds
- Ability to assist Participants with mobility challenges
