Job Description:
Participate as a member of the interdisciplinary team focussing on patient centered care
Work under the direction of the Director of Care (DOC) and Assistance Director of Care (ADOC) in the delivery of care
Embrace and encourage family participation as part of the residents lived experience
to Ensure that residents are being taken care of according to their needs and following any medical instructions from theirphysicians accurately and compassionately
Observe significant changes and reportand updateon residentsoverall conditions status
Initiate the assessment,planning, development and implementation of resident care plans
Supervise Personal Support Workers(PSWs) and other Unregulated Care Providers (UCPs)staff currentlyassigned to your home area
Communicate effectively with the medical care team and other members of the circler of care
Participate in care/situational conferences, medical rounds and team huddles
Providemedication administration and treatment management
Document on all aspects of care and treatments
Comply with compliance and regulatory legislation from the Ministry of Long-term care (MOHLTC)
Provide direction, delegation to PSWs and UCPs
Respond to resident/families concernsand provide immediate actions to mitigate issues
Participate in quality initiative, patient safety and Infection Prevention and Control Practices (IPAC), clinical programs and health and safety practices
Job Requirements:
Current registration with the College of Nurses from a designated Province or State
Relevant long term care experience or experience with seniors/older adults
Experience working with elderly with responsive behaviors or cognitive impairments
Vulnerable Sector Check (with criminal background check)
Fitness Statement
Covid Vaccine(at minimum two doses)
TB skin test within past 6 months
Ability to work as a team and to follow direction
Computer literacy and experience
Current Basic Life support and first aid certification
Able to multi task, organize and manage daily task with multiple residents
Strong leadership and communication skills
Able to critically think in emergency situation
