Job Description
- Identifies learning needs of patient through recognized assessment method and sets objectives for each area where patient education is required.
- Performs individualized assessment to identify educational needs, learning style, barriers and key learner and recognize subtle cues to anticipate potential problems.
- Analyzes the assessment data, develop health education plan, set priorities, involve patient, significant others and multidisciplinary team in implementation when appropriate.
- Performs ongoing assessment and make recommendations to the health care team and the patient/ family to adjust the plan of care as required.
- Provides education related to the disease process, medication, pain management, medical equipment, hygienic care, diet and nutritional monitoring, infection control measures, survival skill, prevention and care of complications.
- Assesses, monitors, and implements treatment plans for patient at various stages of the disease focusing on prevention of complications and therapeutic plan, evaluate the outcome of education and modify as required.
- Documents assessments, interventions including recommendations, discharge plan and goals in designated form according to hospital/departmental standards.
- Participates in the development, planning and evaluation of treatment, education, counseling and health promotion activities of the COMPANY.
- Maintains supplies and equipment needed for the patient education and operate required equipment safely.
