Job Title: Resource Mother
Location: Atlanta GA
Responsible for the North GA region to include Hall White Whitfield and Floyd counties.
Duration: 6 months

Walk me through the daytoday responsibilities of this the role and a description of the project (Outside of Workday JD):
- Position Purpose: Provide support to members participating in Planning for Healthy Babies (P4HB) program that delivers low birth weight infants to ensure they understand and utilize the social service or clinical benefits provided by the plan. Conduct home visits for evaluation of any needs or counsel on direction of care.
- Complete intakesbased interviews with P4HB participants and supporting to identify the needs of the members.
- Identify and document member needs and provide to clinical management to develop plan of care.
- Coordinate and implement the delivery of member care and social services within the community.
- Assist participants in locating and utilizing community resources including legal medical financial assistance and other referral services.
- Coordinate services to assist participants with attending primary care medical appointments.
- Serve as the liaison between participants and family members significant others nurses physicians and organizational components to facilitate communication linkage and continuity of service.
- Document status of appointments enrollment and participation in planned services and benefits.
Required Skills/Experience:
- Previous experience working in the department of family and children services hospital community centers and or physician offices. Provide education to P4HB members on plan services and benefits to encourage healthy behaviors including healthy eating choices and smoking cessation.
- 2 years of experience working with community agency providing services encompassing the basic competency areas of health care managed care member outreach (community resources communication skills individual and family advocacy health education services skills and responsibilities).
- Selfmotivated and able to manage time effectively
Education Requirement:
- Associates degree in social services required and 2 years of experience in a social servicerelated position
Education Preferred:
- Bachelors degree in a social servicerelated field preferred.
