Key Responsibilities:

• Assess patients health needs and develop individualized care plans.
• Coordinate and monitor healthcare services to ensure effective care delivery.
• Provide education and support to patients and their families.
• Collaborate with healthcare providers social workers and other professionals.
• Maintain accurate and uptodate patient records and documentation.

Qualifications:

• Registered Nurse (RN) with a valid license in the UK.
• Previous experience in case management or a related clinical role.
• Strong communication organizational and problemsolving skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of healthcare regulations and best practices.
• Knowledge of CQC regulations and quality assurance processes
