Position summary:
The Care Navigator plays a crucial role in providing support to cancer patients and their caregivers throughout their cancer journey. Serving as the main point of contact for all patients enrolled on the platform they need to build strong relationships with both patients and caregivers to ensure patients remain actively engaged on the app for cancer care
The Care Navigator needs to be committed to breaking down barriers to care. these navigators identify critical resources for patients helping them navigate through various services and offerings. The core responsibility of the Care Navigator is to offer guidance emotional support and care coordination for all the patients. This entails collaborating closely with healthcare teams connecting patients with necessary resources and ensuring a seamless personalized experience throughout their treatment and recovery. This role plays an integral part in enhancing the overall wellbeing of patients and their journey towards positive health outcomes
Qualifications:
Bachelors degree in nursing social work psychology healthcare administration or a related field (Masters degree preferred)
Preferred: Formal education as a cancer care navigator Oncology nursing background
Experience:
Minimum of 2 years of experience working in healthcare in oncology or patient navigation nursing roles home care caregiver roles
Strong understanding of cancer treatments medical terminologies and the emotional challenges faced by cancer patients
Skills:
Excellent interpersonal and communication skills to effectively interact with patients caregivers and healthcare professionals
Empathy compassion and the ability to provide emotional support to patients in distress
Strong organizational skills to manage multiple patients needs and coordinate care effectively
Problemsolving abilities to address logistical challenges and patient concerns
Strong proficiency in English and Hindi writing and speaking skills
Responsibilities:
Patient Onboarding and Engagement:
Guide newly enrolled patients and caregivers through the program setting up their profiles explaining the available features and how to navigate the app effectively
Foster a welcoming and reassuring experience during onboarding emphasizing the benefits of using the app for their cancer journey
Patient Monitoring and Adherence:
Provide anticipatory guidance to patients in coping with outcomes of treatment
Maintain regular communication with patients and caregivers checking in to ensure their continued engagement with the app
Address any concerns or barriers to app usage providing support and guidance to overcome challenges
Acting as a Bridge between Oncologists and Patient:
Facilitate effective communication between oncologists and patients conveying treatment updates answering queries and addressing concerns
Ensure seamless information flow between oncologists and patients for informed decisionmaking
Patient and Caregiver Education:
Provide and reinforce education to patients families and caregivers about side effect management posttreatment care and survivorship
Collaborate with oncologists in recommending appropriate educational content from the apps resources ensuring patients receive accurate and relevant information aligned with their diagnosis treatment and emotional needs
Emotional Readiness and Support:
Provide support by actively listening to patients and caregivers feelings fears and anxieties related to their cancer journey
Offer coping strategies positive reinforcement and referrals to physio and psycho health professionals when needed
Access to Cancer Products and Services:
Guide patients in exploring and buying cancerrelated products such as wigs prosthesis and other supportive services like nutrition counselling emotional counselling and physiotherapy etc. available through the app
Communication and Reporting:
Maintain accurate records of patient interactions and interventions ensuring proper documentation and reporting
Regularly communicate patient feedback to the concerned team
